By completing details at right, We hereby apply for membership in Employers Association, Inc. and agree to subscribe to the Association for the sum of {Choose Membership Type}. Dues are payable on an annual basis at a rate determined by the Board of Directors. Enclosed with this application is a check or credit card information to pay our first year membership dues and a one time $100 initiation fee. We understand that in subsequent years, we will be billed for membership dues on our membership anniversary date.
